In the name of restoring professionalism, an influential group of physician-educators have urged academic medical centers to take the lead in purging the house of medicine of the conflicts of interest created by industry's marketing. I argue that this revivalist movement is misguided, uses “conflict of interest” as an epithet, creates counter-productive incentives, and fails the duty to prepare physicians for ethical engagement with their commercial partners in patient care.

In purity and holiness I will guard my life and my art.Every physician-patient encounter is a conflict of interest. Every physician-payer encounter is also a conflict of interest.Wide-spread criticism of the pharmaceutical industry’s extravagant marketing practices and some doctors’ undignified, even appalling eagerness to stuff themselves, their pockets and their offices with the industry’s “stuff,” prompted physician groups, the drug and device industry itself to institute reforms designed better to limit industry influence on physicians.But according to Troyen Brennan and his (...) coauthors, the reforms, while reducing some of the most egregious instances of industry influence peddling and physicians’ self-debasement, were doomed fundamentally, for three reasons. First, they were based on common sense but incorrect assumptions about the mechanism of industry influence and on myths about the efficacy of disclosure as an ethical disinfectant. Second, they relied on voluntary adherence and so failed to pinpoint responsibility for monitoring compliance and enforcement. (shrink)

A recent New York Times/CBS poll shows that nearly 80 percent of respondents think the American “health care system is headed toward a crisis because of rising costs.” Indeed, the public has become well acquainted with ominous-looking graphs that detail the nation’s health care spending. The increasingly steep slope of the graph showing the percentage of gross domestic product spent on health care invites tongue-in-cheek projections for when health care spending will finally consume it all.High aggregate health care expenditures result (...) from the low prices individuals pay for coverage. On average, patients pay only five cents out-of-pocket for every dollar of hospital charges they generate. The remainder is paid by private or public health insurance. Patients pay slightly more for physicians’ services—nineteen cents on each dollar. For health care of all types, patients pay approximately twenty-four cents on each dollar. With individuals paying what amount to fire sale prices for health care, it should come as no surprise that they purchase a great deal of coverage. (shrink)

The recent focus on conflict of interest seriously misfires by fixating on monetary payoffs while ignoring all the other things that people care about, things that can bias their judgment and lead to wrongdoing. Wrongdoing should be the focus, not the temptations and motivations that sometimes result in it. This essay explores the evidence in support of strong regulations against conflict of interest, and I conclude that corruption fears have resulted in social-distancing policies of drug reps from physicians and they (...) have given us sharply constrained information markets. There is some evidence that this may be disadvantageous to patients. Information market restrictions do promote powerful political and corporate interests. Whether there are compensating benefits in reduced expenditures on drugs and devices remains to be seen. We can only hope that our institutional policies eventually will be revised in a manner that better respects physicians as sophisticated consumers of product marketing and holds them accountable for their choices not their motivational states. In this important new text, Keith Lehrer introduces students to the major traditional and contemporary accounts of knowing. Beginning with the accepted definition of knowledge as justified true belief, Lehrer explores the truth, belief and justification conditions on the way to a thorough examination of foundation theories of knowledge, externalism and naturalized epistemologies, internalism and modern coherence theories as well as recent reliabilist and causal theories. Lehrer gives all views careful examination and concludes that external factors must be matched (...) by appropriate internal ones to yield knowledge. Readers of Professor Lehrer's earlier book _Knowledge_ will want to know that this text adopts the framework of that classic text. But _Theory of Knowledge_ is a completely rewritten and updated version of that book that has been simplified throughout for student use. (shrink)

Contextualism has been hotly debated in recent epistemology and philosophy of language. The Case for Contextualism is a state-of-the-art exposition and defense of the contextualist position, presenting and advancing the most powerful arguments in favor of the view and responding to the most pressing objections facing it.

This paper brings together two positions that for the most part have been developed and defended independently of one another: contextualism about knowledge attributions and the knowledge account of assertion.

This paper defends two main theses related to emerging deepfake technology. First, fears that deepfakes will bring about epistemic catastrophe are overblown. Such concerns underappreciate that the evidential power of video derives not solely from its content, but also from its source. An audience may find even the most realistic video evidence unconvincing when it is delivered by a dubious source. At the same time, an audience may find even weak video evidence compelling so long as it is delivered by (...) a trusted source. The growing prominence of deepfake content is unlikely to change this fundamental dynamic. Thus, through appropriate patterns of trust, whatever epistemic threat deepfakes pose can be substantially mitigated. The second main thesis is that focusing on deepfakes that are intended to deceive, as epistemological discussions of the technology tend to do, threatens to overlook a further effect of this technology. Even where deepfake content is not regarded by its audience as veridical, it may cause its viewers to develop psychological associations based on that content. These associations, even without rising to the level of belief, may be harmful to the individuals depicted and more generally. Moreover, these associations may develop in cases in which the video content is realistic, but the audience is dubious of the content in virtue of skepticism toward its source. Thus, even if—as I suggest—epistemological concerns about deepfakes are overblown, deepfakes may nonetheless be psychologically impactful and may do great harm. (shrink)

This paper brings together two positions that for the most part have been developed and defended independently of one another: contextualism about knowledge attributions and the knowledge account of assertion.

Natural myside bias is the tendency to evaluate propositions from within one's own perspective when given no instructions or cues (such as within-participants conditions) to avoid doing so. We defined the participant's perspective as their previously existing status on four variables: their sex, whether they smoked, their alcohol consumption, and the strength of their religious beliefs. Participants then evaluated a contentious but ultimately factual proposition relevant to each of these demographic factors. Myside bias is defined between-participants as the mean difference (...) in the evaluation of the proposition between groups with differing prior status on the variable. Whether an individual difference variable (such as cognitive ability) is related to the magnitude of the myside bias is indicated by whether the individual difference variable interacts with the between-participants status variable. In two experiments involving a total of over 1400 university students ( n = 1484) and eight different comparisons, we found very little evidence that participants of higher cognitive ability displayed less natural myside bias. The degree of myside bias was also relatively independent of individual differences in thinking dispositions. We speculate that ideas from memetic theory and dual-process theory might help to explain why natural myside bias is quite dissociated from individual difference variables. (shrink)

Keith Oatley draws on theories from psychology, philosophy and linguistics, as well as writings from other social sciences, to show how emotions are central to any understanding of human actions and mental life.

Many critics of dual-process models have mistaken long lists of descriptive terms in the literature for a full-blown theory of necessarily co-occurring properties. These critiques have distracted attention from the cumulative progress being made in identifying the much smaller set of properties that truly do define Type 1 and Type 2 processing. Our view of the literature is that autonomous processing is the defining feature of Type 1 processing. Even more convincing is the converging evidence that the key feature of (...) Type 2 processing is the ability to sustain the decoupling of secondary representations. The latter is a foundational cognitive requirement for hypothetical thinking. (shrink)

Contemporary virtue ethics was revolutionized by Alasdair MacIntyre’s reconfiguration using practices as the starting point for understanding virtues. However, MacIntyre has very pointedly excluded the professions of the financial world from the reformulation. He does not count these professions as practices, and further charges that virtue would actually hinder or even rule out one’s pursuit of these professions. This paper addresses three tasks, in regard to the financial profession of investment advising. First, the paper lays out MacIntyre’s soon-to-be-published charges against (...) the investment world, as specified to this profession. Second, it sets forth the role function of investment advising and shows that it does possess internal goods—the crucial aspect of practices disputed by MacIntyre in this case—including goods of production and goods of performance. Third, it demonstrates that this function, and the goods tied to it, cannot be achieved apart from the virtuous pursuit of the profession, thereby showing that MacIntyre’s charges are mistaken. Investment advising is shown to be a virtue-based practice, and the groundwork is laid for showing that other financial professions are so, as well. (shrink)
